Variation in Lumbar Vertebral Pedicle Structure with Bone Mineral Density

چکیده

Introduction: Despite widespread use of pedicles in lumbar spine fixation, relatively little is known regarding variations in pedicle structure particularly in osteoporosis. It is valuable to understand this relationship because pedicles provide an important area for screw fixation and a point of complication when screws penetrate through weak pedicle walls into adjacent structures. Hirano et al. compared DEXA measurements of bone mineral density (BMD) between cadaveric lumbar spines to define groups of normal and osteoporotic spines. The proportion of pedicle cortical to cancellous bone was determined using peripheral quantitative CT. In the osteoporotic group the pedicle cortex represented a smaller proportion of the pedicle on cross-section. Inceoglu et al. used microCT to evaluate lumbar pedicle cancellous structure and suggested that the trabecular thickness may decrease with osteoporosis while the trabecular number remains constant. However, in this study there were no pedicles from osteoporotic lumbar spines. The objective of the present study is to characterize the variation in lumbar pedicle structure and to clarify how pedicle structure is related to lumbar spine and pedicle BMD.

The Pedicles Are Not the Densest Regions of the Lumbar Vertebrae: Implications for Bone Quality Assessment and Surgical Treatment Strategy

STUDY DESIGN Cadaver study. OBJECTIVE To determine the bone density of lumbar vertebral anatomic subregions. Bone mineral density (BMD) is a major factor in osseous fixation construct strength. The standard region for implant fixation of the spine is the pedicle; however, other regions may be more viable options with higher bone quality. METHODS Using computed tomography images, the spine w...

Cortical bone trajectory for lumbar pedicle screws.

BACKGROUND CONTEXT Achieving solid implant fixation to osteoporotic bone presents a clinical challenge. New techniques and devices are being designed to increase screw-bone purchase of pedicle screws in the lumbar spine via a novel cortical bone trajectory that may improve holding screw strength and minimize loosening. Preliminary clinical evidence suggests that this new trajectory provides scr...

Pullout strength of pedicle screws versus pedicle and laminar hooks in the thoracic spine.

While the biomechanical properties of pedicle screws have proven to be superior in the lumbar spine, little is known concerning pullout strength of pedicle screws in comparison to hooks in the thoracic spine. In vitro biomechanical pullout testing was performed to evaluate the axial pullout strength of pedicle screws versus pedicle and laminar hooks in the thoracic spine with regard to surgical...

Fatigue Performance of Cortical Bone Trajectory Screw Compared With Standard Trajectory Pedicle Screw.

STUDY DESIGN Cadaveric biomechanical study. OBJECTIVE To determine fatigue behavior of cortical bone trajectory (CBT) pedicle screws. SUMMARY OF BACKGROUND DATA Cortical bone trajectory screws have been becoming popular in spine surgery; however, the long-term fatigue behavior of the new CBT screws remains understudied and limitations not well defined. METHODS Twelve vertebrae from six ca...

The effect of vertebral rotation of the lumbar spine on dual energy X-ray absorptiometry measurements: observational study.

OBJECTIVE To estimate how axial rotation of lumbar vertebrae quantitatively affects bone mineral density, as measured by dual energy X-ray absorptiometry in the anteroposterior plane. DESIGN Observational study. SETTING University teaching hospital, Hong Kong. PATIENTS Cadaver lumbar vertebrae (L2 to L4) were removed from four adults. MAIN OUTCOME MEASURES Using dual energy X-ray absorp...
